Chronic diseases require long term commitment from boththe patient and the treating physician for improving theirclinical outcomes. This journey can be made more easier ifa proper communication bridge can be made betweenboth of them. This paper describes an alliterative C-basedflow-chart to describe a Collaborative Conversation Mapfor chronic disease care. It crafts a map which serves as achecklist for chronic health care providers, especially thosedealing with obesity and diabetes.
